Removal of Granulation of Tympanic membrane

Treatment consists of steroid containing antibiotic ear drops and removal of the granuloma tissue by scraping or cauterization. An ear drops alone will often dissolve the granuloma away over two to three weeks. However, the most effective way to resolve the infection is to remove the granuloma. Since there is no involvement of nerve endings in the granuloma, it can be removed without any special precautions. The granuloma usually attaches to the tube by a thin stock and can be removed by applying a small amount of suction. Bleeding can be stopped with a cotton ball placed in the ear canal.

In severe cases, granulation tissue healing requires prolonged treatment with parenteral antibiotics, but the fast absorption of fluoroquinolones in gastrointestinal tract allows milder infections to be treated with a two-week course of oral therapy. Combination of a beta-lactam antibiotic and amino glycoside is also effective Treatment should also include surgical debridement of any granulation or osteitic bone.

The best treatment for keloid is prevention in patients with a known predisposition. Ear Keloids can be nullified by avoiding ear piercing. Any skin problems in predisposed individuals should be treated as early as possible to minimize areas of inflammation. Keloid excision by scalpel excision alone is not effective, as it cause recurrences.

Excision should be combined with preoperative, intraoperative, or postoperative triamcinolone or interferon injections. Surgical removal may cause a larger keloid scar. Keloid on ear lobes is less likely to recur after surgical treatment provided proper precautions and post-op treatment is followed.

Laser surgical excision is the best alternative to conventional surgery for keloid removal. Using Co2 Laser the entire keloid mass is excised followed by kenacort injection .This should be followed by radiotherapy. Intralesional corticosteroids are first-line therapy for most Keloids. Gel sheeting with both hydrogel and silicone scar sheets have been used for the treatment of pain and itching in patients with established Keloids, as well as for the management of evolving Keloids and the prevention at the sites of new injuries. Cryosurgery is most useful in combination with other treatments for Keloids. Pulsed dye laser treatment can be beneficial for Keloids.
